Transition is helping students with disabilities and their families think about their life after school and working to identify long-range goals in order to design the school experience to ensure that students gain the skills and connections they need to achieve these goals. This transitional process includes many layers of planning for the future and should be fully understood by everyone involved, each step of the way. Planning for the future is an investment in a student’s well being!
Transition services are a coordinated set of activities, provided by the school and, when necessary, community agencies, to students with disabilities to promote successful movement from high school to adult life. Transition services are part of a results oriented process that focuses on improving both the functional and academic needs of a child with a disability
